Home Health: CAHPS Survey Memo
This memo may be distributed to your patients & their family members in order to prepare them for the survey and help them understand the importance of participating.
We have also included the phone number that will come up on caller ID when we are calling. This should help your patients feel safe to answer the phone since they will be able to identify when we are calling. The more you emphasize this telephone number, 1-877-308-3844, to patients and that you want them to take the survey, the better your response rate will be.
You may print the memo on your letterhead, sign it with an official signature, and include this memo with admission paperwork and distribute to current patients. It is available in English, Spanish and Russian.
